Audit-ready documentation checklist
Evidence on hand, all year.
The inventories, diagrams, policies, and records examiners ask for, and how often to refresh each one.
Audit-ready documentation checklist
Examiners, insurers, and clients tend to ask for the same kinds of evidence. When it is organized and current, a review is a short conversation. When it is not, it becomes a scramble. Use this checklist to see what you have, assign an owner, and set a refresh schedule.
Know what you have
- Asset inventory of computers, servers, network devices, and applications (review quarterly)
- Network diagram showing how systems connect, including remote access and cloud services (review when anything changes, and yearly)
- Data inventory showing where sensitive information lives (review yearly)
- Vendor and third-party list, with what each can access (review yearly)
Policies and procedures
- Acceptable use policy (review yearly)
- Access control and account management policy (review yearly)
- Incident response plan (review yearly, and test)
- Business continuity and disaster recovery plan (review yearly, and test)
- Vendor management procedure (review yearly)
- Policy for AI tools (review every six months)
Proof that it is happening
- Patch and update records (monthly)
- Access reviews, including who has administrator rights (quarterly)
- Backup reports and restore test results (backups monthly, restores at least twice a year)
- Security awareness training records (yearly, with new hires in their first week)
- Security testing results, such as vulnerability scans or penetration tests, and how findings were fixed (as scheduled)
- Physical security records, such as camera and door access reviews, where they apply (quarterly)
Keep it living
- Name an owner for each item. When everything is everyone’s job, it is no one’s.
- Put refresh dates on the calendar. Do not rely on memory.
- Store it in one place. Use a shared folder with access limited to those who need it.
- Practice a mock review. Ask someone who is not involved day to day to request documents the way an examiner would. Anything you cannot produce quickly is your preparation list.
Pair it with a regular review
Many organizations review this checklist at each quarterly business or technology review, so evidence stays current and nothing piles up before a deadline.
